• Please do not mail this form to the National Passport Center but to  APVI. (Address provided after Step 3 with order confirmation number)
• After completing and printing this form, please close the window and return to the passport instructions

Print two duplicate originals of APVI letter of authorization (LOA). The letters have to be manually signed in blue ink.

The applicant's signature on the two (LOA) authorizes us to process the passport application through the Passport Agency.

The two passport photos have to meet Passport Agency Regulations.

                 Where can I have my passport photos Taken?

Your valid US passport must have at least one year of validity left. If your passport is about to expire (less than one year of validity), you will need to renew it before you appy for a second valid passport.

If your passport expires in less than one year, call APVI: 1-866-848-2784 for assistance.

You have to write a letter addressed to the "Department of State" requesting a second valid passport. The letter must explain in detail the reason for needing a second valid passport.

If you are applying for Same-Day or 24-Hour Service, you must provide two copies of one of the following:

• A copy of your plane ticket, or a travel itinerary generated by a travel agency showing your name and date of travel.
• Business traveler's may provide a business letter if neither of the above is available. Click Here for a sample Business letter.

Write one check or money order for $135 payable to: "US Department of State". Write your Date of Birth on the memo line.
