A   Complete New Passport Application Form (DS-11)

  • Please do not sign this form now. (Explained in Step 2)
  • Please do not mail this form to the National Passport Center but to  APVI. (Address provided after Step 4 with order confirmation number)
  • After completing and printing this form, please close the window and return to the passport instructions.


B   APVI Letter of Authorization

Print two duplicate originals of APVI letter of authorization (LOA). The letters have to be manually signed in blue ink.

                                    APVI (LOA)          

The applicant's signature on the two (LOA) authorizes us to process the passport application through the Passport Agency.


C   Two Passport Photos

The two passport photos have to meet Passport Agency Regulations.

D   Proof of Identity

You must provide 2 copies of your valid driver's license or state ID card, issued more than 6 months ago.

If your ID was issued less than six months ago, or you don't have one at all, please click here for additional proof of Identity.


E   Proof of US Citizenship

Provide one of the following:

Certified US Birth Certificate or your most recent US Passport. Note: Mutilated, altered, or damaged passports are not acceptable evidence of U.S. Citizenship.

F   Evidence of Travel

You must provide two copies of one of the following:

  • A copy of your plane ticket, or a travel itinerary generated by a travel agency showing your name and date of travel.
  • Business travelers may provide a business letter if neither of the above is available. Click here for a Sample Business Letter.


G   Government Fees

Write one check or money order for $135 payable to: "US Department of State". Write your Date of Birth on the memo line.

Write a second check or money order for $25 payable to the Court House or Post Office where your passport application will be authorized (explained in Step 2).

  • All personal checks must have a printed address.
  • The government does not accept credit card payment for these fees.
