APVI Letter of Authorization
|
Print two duplicate originals of APVI letter of authorization (LOA). The letters have to be manually signed in blue ink.
APVI (LOA)
The applicant's signature on the two (LOA) authorizes us to process the passport application through the Passport Agency. |

D
One Year Extendable Passport
|
| If your passport is extendable, it will not contain an amendment in the back stating that it is not extendable. |
|

E
Proof of US Citizenship
|
Provide one of the following:
Certified US Birth Certificate or expired 10-year US Passport. Note: Mutilated, altered, or damaged passports are not acceptable evidence of U.S. Citizenship.

Proof of Identity
|
You must provide 2 copies of your valid driver's license or state ID card, issued more than 6 months ago.

| If you are applying for Emergency or Urgent Service (1-4 days processing), you must provide two copies of one of the following:

Lost Statment (If applicable)
|
If the last passport issued to you prior to the 1 year limited passport was lost or stolen you must provide a written signed statment explaining the circumstances surrounding the loss. |

Government Fee
|
Write one check or money order for $142* payable to: "US Department of State." (Write your Name and Date of Birth on the Memo Line.) If you do not provide a check for this fee, the credit card that you provide in Step 3 will be charged. *Starting July 11, 2010, this is the new State Department fee. |
